All patients are screened financially using the at/below 200% federal income poverty guidelines shared by all free clinics and pharmacies in the Union County and Anson County areas. "example: Family of 4 can make no more than $53,000."
Patients must complete an application packet which includes medical and financial questions.

We do not complete disability paperwork.
Pregnancy care is not provided at the Community Health Services of Union County, Inc. clinic.
Narcotic pain prescriptions are not prescribed at our clinic or kept on site.
After you have completed your paperwork, you may call the clinic to schedule an appointment to be screened for eligibility. Your application will then be reviewed and after it is determined that you qualify, you will be contacted to schedule an appointment.
We request a $20.00 donation for each Doctor visit (Cash only please). If your financial situation is dire, please notify the staff. We will not deny services to patients due to financial difficulties.
