All patients are screened financially using the federal poverty guidelines.


Patients must complete an application packet which includes medical and financial questions.

Patients at Community Health Services of Union County, Inc. are treated by:

  • Primary Care Physicians/Providers
  • Physicians and Educators specializing in diabetes, hypertension, general medicine and wellness.
  • Diabetic Nurse Educators.
  • We do not complete disability paperwork. 

  • Pregnancy care is not provided at the Community Health Services of Union County, Inc. clinic.

  • Narcotic pain prescriptions are not prescribed at our clinic or kept on site.

To qualify for this service:

  • Applicants must be 18 years of age and up.
  • Applicants must have no health insurance of any kind (No Medicaid, Medicare, ACA, Tricare, or private insurance.)
  • Applicants must have no physician.
  • Applicants must not have gestational diabetes and must not be pregnant.
  • Applicants must be Union or Anson County residents in North Carolina or Chesterfield and Lancaster County Residents in South Carolina.
After you have completed your paperwork, you may call the clinic to schedule an appointment to be screened for eligibility. Your application will then be reviewed and after it is determined that you qualify, you will be contacted to schedule an appointment.
